Introduction

Tuberculosis (TB) Lung is a highly infectious and fatal disease caused by mycobacterium tuberculosis bacteria, which can attack the lungs and other organs. This disease is a serious public health problem, especially in areas with high transmission levels. The World Health Organization (WHO) has reported that TB is one of the top 10 causes of death worldwide, and it is estimated that 1.5 million people die from TB each year. In Indonesia, TB is a major public health concern, and it is estimated that there are over 1 million new cases of TB each year. This study aims to analyze the influence of social and environmental factors on the incidence of pulmonary TB among patients who have home contact, who were treated at the Medan Lung Special Hospital (RSKP) in 2018.

Background

TB is a disease that can be prevented and treated, but it requires a comprehensive approach that involves not only medical treatment but also social and environmental interventions. The social and environmental determinants of TB include factors such as poverty, lack of education, poor living conditions, and lack of access to healthcare services. These factors can contribute to the spread of TB by increasing the risk of transmission and reducing the effectiveness of treatment. In Indonesia, the prevalence of TB is high, and it is estimated that over 50% of the population lives in poverty. This study aims to investigate the relationship between social and environmental factors and the incidence of pulmonary TB among patients who have home contact.

Methodology

This study uses a case-control design with a number of samples of 120 participants divided into 60 cases and 60 controls. The case consists of pulmonary TB patients who are confirmed positively and are the second member in one family who has a history of home contact with TB sufferers. Meanwhile, control is taken from a patient who is confirmed negative with the same history. Sampling was carried out using the Consecutive Sampling Method at Medan RSKP in October 2018.

Data obtained through interviews using questionnaires and observations. Data analysis was carried out by simple logistics regression tests for bivariate analysis, as well as multiple logistics regression tests for multivariate analysis. The results showed a significant relationship between several determinants and the events of the home contact pulmonary TB.

Research Findings

The results of the analysis revealed that there was a significant effect of the following factors on the events of home contact pulmonary TB:

Knowledge (p = 0.001)

Knowledge of pulmonary TB is the most dominant factor that affects events with an odds ratio (OR) value of 7,560. This shows that individuals who lack knowledge about TB are more at risk of contracting. Lack of knowledge about TB can lead to a lack of understanding of the disease and its prevention, which can increase the risk of transmission. Therefore, education and awareness campaigns are essential to increase public knowledge about TB and its prevention.

Education (p = 0.002)

Low level of education is related to the low understanding of disease and its prevention, thus triggering an increased risk of TB. Education is a critical factor in the prevention and control of TB. Individuals with higher levels of education are more likely to have a better understanding of the disease and its prevention, which can reduce the risk of transmission.

Behavior/Custom (p = 0.019)

The habit of the community in maintaining health, including environmental cleanliness and healthy lifestyle, also contributes to the risk of TB transmission. Behavior and customs can play a significant role in the prevention and control of TB. Individuals who practice good hygiene and maintain a healthy lifestyle are less likely to contract TB.

Other Factors

Other factors that were found to be significant in the analysis include:

Poverty (p = 0.05): Poverty is a significant risk factor for TB. Individuals who live in poverty are more likely to have a lower level of education, poor living conditions, and limited access to healthcare services, which can increase the risk of transmission.
Access to Healthcare Services (p = 0.01): Access to healthcare services is a critical factor in the prevention and control of TB. Individuals who have limited access to healthcare services are more likely to contract TB.

Recommendation

Given knowledge as the most dominant factor, it is recommended that the Health Office conducts education and education campaigns regarding pulmonary TB. This counseling must aim to increase public knowledge about how to prevent TB transmission, the importance of a healthy lifestyle, and early detection of disease. In addition, improving the quality of education and awareness of health in the community is very important to reduce the incidence of TB in the future.

With the right precautions, it is hoped that the incidence of pulmonary TB among home contacts can be minimized and the community can live healthier. The findings of this study highlight the importance of social and environmental interventions in the prevention and control of TB. By addressing the social and environmental determinants of TB, we can reduce the risk of transmission and improve the effectiveness of treatment.

Conclusion

This study aimed to analyze the influence of social and environmental factors on the incidence of pulmonary TB among patients who have home contact, who were treated at the Medan Lung Special Hospital (RSKP) in 2018. The results showed a significant relationship between several determinants and the events of the home contact pulmonary TB. The findings of this study highlight the importance of social and environmental interventions in the prevention and control of TB. By addressing the social and environmental determinants of TB, we can reduce the risk of transmission and improve the effectiveness of treatment.
